If you want to contact your survey's respondents, you should definitely include email or name fields. But this could appear intrusive if your aim is only to gather on-point market data and you are already addressing your surveys to a strictly selected focus group of customers. For cases when your survey is located in the Contact Us page and everyone can fill it in to connect with your Sales or Customer Care, contacts field could be optional.

Use neutral, greenish or bluish colors for your feedback survey, or choose a light gray palette. Your customers could have negative feedback, and in this case, too blatant, flashy colors could irritate a person. On the contrary, for cases when you're only asking for opinions about some new product or feature, a brassy, colorful design with more saturation would do greatly. Remember to keep a good contrast between the background and text, so that your Survey would be accessible to wider audiences.
