If you want to contact your survey's respondents, you should definitely include email or name fields. But this could appear intrusive if your aim is only to gather on-point market data and you are already addressing your surveys to a strictly selected focus group of customers. Use neutral, greenish or bluish colors for your feedback survey, or choose a light gray palette. Your customers could have negative feedback, and in this case, too blatant, flashy colors could irritate a person. On the contrary, for cases when you're only asking for opinions about some new product or feature, a brassy, colorful design with more saturation would do greatly.
